Re: Referrals for Right From the Start and the WV Healthy Start/HAPI Project
(Prenatal Women, Infants, and Postpartum/Interconceptional Women)
As most of you have probably heard by now, the Regional Lead Agency for Right From the Start (Region 7), has temporarily closed and all functions of this office are being managed at the state level. Until this issue can be resolved, the WV Healthy Start/HAPI Project has taken over the management of all referrals for Right From the Start and the HAPI Project.
In the past, many of our Obstetric and Pediatric offices were faxing the Prenatal Risk Screening Instrument (PRSI) and Alternate Entry Form directly into the Regional Office and the Regional Care Coordinator (previously Charlita Atha and Carla Bailey) to make sure referrals were completed in a timely manner. Until further notice, when you fax the PRSI or Alternate Entry Referral Form to the state office (Office of Maternal, Child and Family Health), if you would also include the HAPI Project Office Fax number we will make sure that your patients get referred to a Designated Care Coordinator for RFTS and HAPI in a timely manner. You may also refer a postpartum woman for HAPI Project services by using the Alternate Entry Form or simply calling us with referral information.
