Veterans Choice Program-WV
Recently a lot of you have had a patient that presented a fairly new insurance program for Veterans called Veterans Choice Program. This coverage is only effective IF the patient is more than 40 miles from the nearest VA Hospital and are unable to obtain an appointment within the next 30 days.
This coverage is NOT eligible for Emergency Room visits and not for an inpatient admission or clinic visit.
Before your Veterans Choice Card can be used, you must first meet the following test of eligibility for
Choice Card benefits. If, after following Step 1 of the test you are not eligible for benefits, just hold
on to your Card – you do not need to call us.

Before you call us, ask yourself these four simple questions:
1) Have you been told by your local VA medical facility that you will need to wait
more than 30 days from your preferred date or the date medically determined
by your physician? OR
2) Is your current residence more than 40 miles drive distance from the closest
VA health care facility? OR
3) Do you need to travel by plane or boat to the VA medical facility closest
to your home? OR
4) Do you reside in a location that is 40 miles drive distance or less from a VA
medical facility and face an unusual or excessive burden in traveling to such
a facility?

Choice Card. VERIFY YOUR ELIGIBILITY by calling 866-606-8198.
When you call us, please be prepared to provide us with any other health
insurance coverage you have, such as employer or union-provided health plans,
so we can assess coverage responsibilities.

(Prenatal Women, Infants, and Postpartum/Interconceptional Women)
As most of you have probably heard by now, the Regional Lead Agency for Right From the Start (Region 7), has temporarily closed and all functions of this office are being managed at the state level. Until this issue can be resolved, the WV Healthy Start/HAPI Project has taken over the management of all referrals for Right From the Start and the HAPI Project.
In the past, many of our Obstetric and Pediatric offices were faxing the Prenatal Risk Screening Instrument (PRSI) and Alternate Entry Form directly into the Regional Office and the Regional Care Coordinator (previously Charlita Atha and Carla Bailey) to make sure referrals were completed in a timely manner. Until further notice, when you fax the PRSI or Alternate Entry Referral Form to the state office (Office of Maternal, Child and Family Health), if you would also include the HAPI Project Office Fax number we will make sure that your patients get referred to a Designated Care Coordinator for RFTS and HAPI in a timely manner. You may also refer a postpartum woman for HAPI Project services by using the Alternate Entry Form or simply calling us with referral information.

WV DMV Medical Report Form
If your family member’s driving ability appears to be impaired due to age, disability or other health problems, please consider requesting a WV DMV Medical Review on their behalf.
When drivers are reported, the DMV will review all information recieved. The driver may be required to see a licensed physician or optometrist for a check-up or take the road skills test to demonstrate their ability to drive safely. As well, periodic medical and/or visual examinations and/or driver re-examination may be required of drivers who have been evaluated and deemed “at risk” by the DMV. All cases are reviewed on an individual basis to determine the appropriate course of action.
Anyone can report concerns about an unsafe driver but it will NOT be anonymous. A DMV employee, a physician, a caretaker or a member of their immediate family. If a law enforcement officers encounters and personally observes a potentially indafe driver, they must report it to the DMV. Immediate family members include a:
- Spouse
- Sibling
- Parent
- Son or Daughter
- Caretaker
- One who has been appointed as power of attorney. They must supply our office with the proper documentation.
